Common All-on-X Prosthetic Problems: Screws, Chips, Wear and Repairs

Full-arch implant prostheses are designed to function as a single restorative system, but mechanical problems can still occur over time. Understanding the difference between an implant problem and a prosthetic problem helps patients know when to seek care and what may be repairable.

Loose Prosthetic Screws

A loose screw can cause clicking, movement or a change in the bite. It should be evaluated promptly. Continued chewing on a loose component can increase stress on the prosthesis and other implants.

Chipped or Fractured Provisional Teeth

PMMA and other provisional materials are intentionally more repairable than many definitive ceramics. Chips or fractures can occur during healing, especially with heavy bite forces or parafunction.

A provisional fracture does not necessarily indicate implant failure, but it should be repaired and the bite evaluated.

Zirconia Chips or Fractures

Zirconia is highly wear-resistant, but no ceramic is indestructible. Fracture risk depends on material thickness, prosthetic design, implant support, connector dimensions, occlusion and parafunction.

Wear of Opposing Teeth or Restorations

The opposing dentition matters. Natural teeth, crowns, dentures and another full-arch prosthesis can each create a different functional environment. Surface finish and occlusal design are important when managing wear.

Food Trapping Under the Prosthesis

Food can collect beneath a fixed bridge. Some degree of cleaning access is necessary, but excessive food trapping may indicate that the tissue-side contour or hygiene plan needs reevaluation.

Bite Changes

The bite can change as tissues heal, opposing teeth move or restorative components wear. A patient who suddenly feels that one area hits first should have the prosthesis evaluated.

Fractured Access-Hole Fillings

The small restorative material used to seal screw-access openings can wear or dislodge. This is generally a prosthetic maintenance issue rather than an implant failure.

When Does the Prosthesis Need to Be Removed?

Removal may be needed for certain repairs, screw replacement, implant evaluation, deep cleaning or prosthetic modification. It is not automatically required at every routine visit.

How Do You Know if the Problem Is the Implant or the Bridge?

Symptoms can overlap. Movement, pain, swelling, drainage, fracture or bite changes should be evaluated clinically and sometimes radiographically.

Can Most Prosthetic Problems Be Repaired?

Many can, depending on the material and extent of damage. Provisional restorations are generally easier to modify. Definitive zirconia repairs may require a different strategy or laboratory involvement.

How Can Patients Reduce Mechanical Problems?

  • avoid chewing extremely hard objects;
  • follow postoperative diet instructions during healing;
  • report bite changes early;
  • use a protective appliance if recommended for grinding;
  • maintain regular professional evaluation;
  • do not ignore clicking, movement or repeated screw loosening.

Why Repeated Screw Loosening Matters

A screw that repeatedly loosens may be a symptom of a larger mechanical issue such as occlusal overload, prosthetic misfit, cantilever stress or component wear. Repeatedly tightening the screw without evaluating the cause may not solve the underlying problem.

Not every All-on-X problem is an implant failure. Loose screws, chips, wear, food trapping and bite changes are often prosthetic or mechanical issues that can be evaluated and managed separately from the implants themselves.
